New Patient Forms

If you would like to complete your patient forms prior to your appointment, you may do so through your patient portal. Login information for your portal was sent via text message from our system, “Klara,” either on the day your appointment was scheduled or the following day. If you are unable to locate the text message, please text us at (602) 835-4485 with your full name and date of birth and let us know you would like assistance accessing your patient portal to complete your new patient forms. You may also call our office at (480) 899-7546 to speak with a staff member for assistance.
